    The 2010 McDonald criteria for the diagnosis of multiple sclerosis are widely used in research and clinical practice. Scientific advances in the past 7 years suggest that they might no longer provide the most up-to-date guidance for clinicians and researchers. The International Panel on Diagnosis of Multiple Sclerosis reviewed the 2010 McDonald criteria and recommended revisions. The 2017 McDonald criteria continue to apply primarily to patients experiencing a typical clinically isolated syndrome, define what is needed to fulfil dissemination in time and space of lesions in the CNS, and stress the need for no better explanation for the presentation. The following changes were made: in patients with a typical clinically isolated syndrome and clinical or MRI demonstration of dissemination in space, the presence of CSF-specific oligoclonal bands allows a diagnosis of multiple sclerosis; symptomatic lesions can be used to demonstrate dissemination in space or time in patients with supratentorial, infratentorial, or spinal cord syndrome; and cortical lesions can be used to demonstrate dissemination in space. Research to further refine the criteria should focus on optic nerve involvement, validation in diverse populations, and incorporation of advanced imaging, neurophysiological, and body fluid markers.

The 2017 revisions of the multiple sclerosis diagnostic criteria1 provide an opportunity for reflection.

    When asked by a trainee how he knew that an acutely ill patient had multiple sclerosis, Harvard neurologist Raymond D Adams quipped: “How does a farmer know his sheep?” The parallel between a farmer's intuition and a clinician's judgment was apt. This anecdote is from the mid-1970s, when no therapies for multiple sclerosis existed, and clinical diagnosis relied on recognition of the phenotype and exclusion of mimics. Direct tissue examination, usually at autopsy, was the true gold standard for diagnosis.

    In an era of evidence-based medicine, the value of rehabilitative therapies in Parkinson's disease has been difficult to establish. Although experts in movement disorders routinely aver the merits of, and recommend, such interventions to their patients, randomised trials have often yielded contradictory, few, or non-generalisable results (eg, as evident in the systematic review1 of the effectiveness of Nordic walking in Parkinson's disease). The reasons for such heterogeneity of conclusions have been extensively discussed,2 but the remedies are not self-evident.

    Causes of post-stroke epilepsy are manifold and not just restricted to the stroke itself (primary post-stroke epilepsy). These causes range from premorbid conditions already present before the stroke, to morbidity that develops after the stroke, either as a complication of, or independent of, the stroke. Seizures associated with a pre-existing disposition (eg, meningitis, head trauma, or encephalopathy caused by, for example, diabetes or hypertension) can be triggered by stroke or by co-pathologies developing after a stroke (secondary post-stroke epilepsy).

    The field of multiple sclerosis has undergone astounding changes over the past three decades. Many developments are the result of MRI, which has improved multiple sclerosis diagnosis and enhanced clinical trial outcome measures. The term clinically isolated syndrome describes a first symptomatic episode of CNS demyelination that might later develop into multiple sclerosis. Improving the imaging criteria to predict conversion to clinically definite multiple sclerosis has been a major, collaborative research effort.

    Alzheimer's disease is characterised by progressive pathophysiological changes that correspond roughly to preclinical (ie, cognitively unimpaired), mild cognitive impairment, and dementia stages.1 Biomarkers continue to be developed, tested, and used to detect and track the pathophysiological manifestations of this disease. Although neuropsychological tests, clinical ratings, and composite cognitive test scores have shown initial promise,2–5 the field would benefit from additional ways to detect and track cognitive, behavioural, and functional manifestations of Alzheimer's disease with optimal power, ease, and availability, particularly in the preclinical and early clinical stages.

    Most intracerebral haemorrhages are due to cerebral small vessel diseases hypertensive arteriopathy (arteriolosclerosis), which affects deep perforating vessels, and cerebral amyloid angiopathy (CAA), which affects superficial cortical and leptomeningeal vessels. Diagnosis of CAA is important because it has a high recurrence risk (7·4% per year in a pooled analysis of cohort studies)1 and might require specific prevention strategies (eg, modifying the use of antithrombotic drugs) or, in the future, disease-modifying treatments targeting vascular β-amyloid.

    Your Editorial highlighted the need of trials on dementia prevention, and welcomed the launch of World Wide Fingers WW-FINGERS),1 but did not provide details on this initiative. WW-FINGERS springs from the successful experience of the Finnish geriatric intervention study to prevent cognitive impairment and disability (FINGER),2 a landmark trial that showed that a multidomain lifestyle intervention, based on simultaneous management of several vascular and lifestyle-related risk factors for dementia and Alzheimer's disease (AD), can benefit cognition in people aged 60–77 years with high risk of dementia.

  • Subthalamotomy for Parkinson's disease: back to the future?

    MRI-guided focused ultrasound is a novel method for creating thermic ablations in the thalamus or basal ganglia without opening the skull. The method uses focused sonic energy and real-time MRI thermography of brain temperature to control lesion size and location precisely. This is a principle advantage over gamma-knife radiosurgery, which is another incision-free lesioning technique but induces radionecrosis that is difficult to control and develops within months of the procedure. In July, 2016, the US Food and Drug Administration approved MRI-guided focused ultrasound for unilateral thalamotomy for treatment of medically refractory and disabling essential tremor.

    Chronic inflammatory demyelinating polyneuropathy (CIDP) is the most common chronic autoimmune neuropathy and often responds to corticosteroids, plasmapheresis, and intravenous immunoglobulin (IVIg).1 The only US Food and Drug Administration-approved, and the most commonly applied therapy, is IVIg, which, at 2 g/kg followed by 1 g/kg every 3–4 weeks, has been shown to prevent relapses and maintain stability in 75% of patients.2 Subcutaneous immunoglobulin (SCIg), an alternative route of IgG administration, is approved for IgG replacement therapy in primary immunodeficiency,3 but whether or not it has an effect in balancing immune dysregulation in autoimmune diseases, as IVIg does,4 has not been formally tested.

    Viruses are the most commonly identified causes of meningitis and encephalitis, but in as many as half of patients, particularly those with encephalitis, the cause is not identified. Hasbun and colleagues1 examined the epidemiology of meningitis and encephalitis in 26 429 adults in the USA between 2011 and 2014. The most common causes were enterovirus (52%), unknown causes (21%), bacterial meningitis (14%), herpes viruses (8%), non-infectious causes (4%), fungi (3%), arboviruses (1%), and other viruses (1%).

Selecting just a few studies to highlight is quite challenging because many has been reported, from observations of innovative therapeutic approaches in animal studies to the results of successful clinical trials. Perhaps the most exciting of these advances have been in Duchenne muscular dystrophy and spinal muscular atrophy.
