Putting deep brain stimulation for depression in a wider perspective.

Deep brain stimulation (DBS) is an evidence-based treatment for Parkinson's disease and is experimentally used for depression with around 50% of patients showing a good effect. DBS involves the neurosurgical implantation of electrodes in specific brain areas, which modulate surrounding neural tissue with electrical pulses. One of the attractive features of DBS is its flexibility, as therapeutic effect can be optimised by changing the electrical properties of the pulses, such as contact selection, amplitude, frequency, and pulse width. In studies of DBS for Parkinson's disease, systematic exploration of these different properties has yielded essential insight into how different settings affect different symptoms. So far, studies in DBS for depression have understandably adopted parameter optimisation strategies from these neurological studies, which usually fix the pulse width at 90 μs, frequency at 130 Hz, and increase amplitude in a step-wise manner. However, not only is depression a different disorder with different implicated pathological brain circuits than Parkinson's disease, but DBS is also targeted at different brain areas. It is, therefore, conceivable that effective parameters for depression other than amplitude are overlooked.
