We end another year in dementia research without disease modifying treatments for Alzheimer's disease and other related dementias. Despite this disquieting fact, we start the new year with exciting and unprecedented momentum. Avenues of scientific investigation are rapidly expanding and span a vast array of basic and clinical science. To present the most important advances in dementia research in 2019 is a daunting challenge! I chose to concentrate on research not only with compelling science, but also that embraces topics that I believe will continue to be pivotal in the new decade, with a focus on the apolipoprotein E ( APOE) gene, neurodegeneration, inflammation, and the brain vasculature. I end with an encouraging study of the only actionable defense against dementia—prevention.
