A 66-year-old male patient with a history of right-sided pneumonectomy for lung cancer and persistent atrial fibrillation (AF) presented with symptomatic bradycardia–tachycardia episodes. The patient had undergone repeat pulmonary vein isolation and extensive left atrial substrate ablation for treatment of AF-mediated cardiomyopathy. At admission, the patient was in sinus rhythm with narrow QRS complex and normalized left ventricular systolic function. We opted for pacemaker therapy with His bundle pacing (HBP) and subsequent antiarrhythmic drug treatment or potential atrioventricular nodal ablation.
