Bariatric surgery and type 2 diabetes: a step closer to defining an optimal approach.

Substantial and durable weight loss can lead to remission of type 2 diabetes for some individuals who are overweight or obese, with huge potential health benefits. In The Lancet Diabetes & Endocrinology, Dag Hofsø and colleagues report results from the Oseberg study, which take us an important step closer to realising this potential. In a well-designed randomised controlled trial, the investigators show that gastric bypass is associated with a higher remission rate of type 2 diabetes than sleeve gastrectomy at 1 year after surgery. The gastric bypass group showed greater weight loss and a higher reduction of total body fat. Both groups showed similar improvement of β-cell function. Based on evidence from several randomised controlled trials, weight loss must now be considered a key component of therapy for overweight or obese people with type 2 diabetes. In the DiRECT study, which assessed the effectiveness of a primary care-based weight-loss intervention involving an intensive dietary programme, a linear relation between weight loss and improvement of glycaemic control was reported.
