To the Editor We commend the authors of “Association of Duration and Type of Surgical Prophylaxis With Antimicrobial-Associated Adverse Events” on their investigation efforts into an important topic of discussion.¹ They retrospectively reviewed the Veterans Affairs Surgical Quality Improvement Project Database to assess postoperative 30-day surgical site infections (SSIs), 7-day incidence of acute kidney injury (AKI), and development of Clostridium difficile infection at 90 days after cardiac, vascular, colorectal, and orthopedic surgery. They concluded that longer duration of prophylactic antibiotics did not lead to decreased rate of SSI but rather increased rates of AKI and C difficile infection.
