Background and Aims: Diabetic nephropathy is one of the most important complications of diabetes mellitus. Recently, it has been reported that diabetics with sarcopenia have a high incidence of nephropathy. Five times sit-to-stand was developed as a measure of lower extremity strength, but it has been evaluated as physical function. However, the association between of physical function with microalbuminuria has been still unknown. Thus, we evaluated five times sit-to-stand and investigated its association with microalbuminuria in a cross-sectional study. Materials and Methods: This cross-sectional study recruited T2DM patients attending Ayabe City Hospital. Urinary albumin-to-creatinine ratio (UACR) was obtained by spot urine exams. We assessed the association of five times sit-to-stand with log UACR with linear regression model. In the multivariate model, we used age, gender, height, skeletal muscle mass, grip strength, body fat mass, systolic blood pressure, diastolic blood pressure, hemoglobin A1c, smoking status and alcohol as co-variates. Results: Among 4patients (269 men, 66.4 %) , mean age, body mass index, hemoglobin A1c, five times sit-to-stand, and median UACR were 68.5 ± 11.1 years, 23.7 ± 3.8 kg/m2, 7.1 ± 0.8 %, 8.4 ± 2.2 seconds, and 12.7 mg/gCr respectively. Log UACR was associated with five times sit-to-stand in an univariate model (r = 0.19, P < 0.001) , and in a multivariate model (ß = 0.12, P = 0.035) . Conclusion: Five times sit-to-stand was associated with UACR in Japanese patients with type 2 diabetes. The important relationship between physical function and diabetic nephropathy was shown.
