Aneurysm wall enhancement (AWE) as seen on high-resolution vessel wall imaging (VWI) sequences has been an interesting yet controversial topic within recent years. Some studies have proposed that the presence of AWE may be a harbinger to aneurysm instability and rupture, whereas the lack of enhancement may indicate a lower likelihood of impending rupture. If these relationships were to be true, VWI of unruptured intracranial aneurysms would likely be a useful tool in risk stratification and deciding whether or not an aneurysm requires intervention. This is particularly true in cases of clinical equipoise where the decision to treat does not surely meet criteria for intervention vs observation. Furthermore, VWI could be useful in monitoring changes in unruptured aneurysms that are followed over time....
