This special edition of the European Journal of Clinical Investigation is timely. Developmental programming is now seen as a major cause of life-course disease and hence a major target for successful health interventions. Research into programming challenges, mechanisms, outcomes and interventions thus has great clinical, economic and sociological relevance. There were hints of developmental programming mechanisms in studies conducted by Barraclough sixty years ago.
