Chemical neurolysis of the lateral branches of the sacral dorsal rami for the treatment of chronic pain in the sacroiliac joint: case report and description of the technique.

The posterior portion of the Sacroiliac Joint (SIJ) is innervated by the lateral branches (LBS) of the dorsal ramus of the spinal nerves from S1-S3, with possible contributions from L4 and L5 branches. Many different fluoroscopy techniques for radiofrequency ablation of these (LBs) have been performed for SIJ pain when conservative treatment is not effective. Our current understanding on SIJ posterior innervation shows a Posterior Sacral Network (PSN) formed by these LBs that converges over the lateral sacral crest at the levels between S1-S3. Moreover, chemical neurolysis is a lower-cost option compared to radiofrequency ablation. The purpose of these case reports is to propose a new ultrasound-guided with fluoroscopy verification technique for chemical denervation of the PSN.
