Morphological differences between dementia with Lewy bodies and Parkinson's disease‐dementia

Dementia with Lewy bodies (DLB) and Parkinson's disease‐dementia (PDD) are two major neurocognitive disorders in the spectrum of Lewy body dementia (LBD) that overlap in many clinical and neuropathological features, although they show minimal differences [1]. Their morphology is characterized by a variable combination of Lewy body disease (LBD) and Alzheimer disease (AD)‐related pathologies, the latter usually being more severe in DLB [2,3]. Cerebral amyloid angiopathy (CAA) is very common in AD with a prevalence up to 97% [4]. Recently, Hansen et al [5], comparing 52 autopsy cases of PDD and 16 DLB cases, showed that CAA was more common and more severe in DLB than in PDD (p=0.003 and 0.009, respectively), with significantly higher CAA scores in the occipital and parietal lobes.
