Abstract Background For transgender and/or gender non-conforming (TGNC) individuals who already experience health disparities, modern medical advancements such as transplant medicine can outpace progress in healthcare equity. As both gender-affirming treatments and organ transplantation become increasingly accessible, TGNC individuals at this intersection require sensitive assessments by mental health providers to mitigate health inequities. Case Presentation In this case report, we trace ways in which inadequate understanding of TGNC health and broader social disparities interacted during the evaluation of a transgender living organ donor. Conclusions We argue that mental health providers play an essential role in advocating for greater health equity in TGNC and transplant medicine.
