The Institutional Embedding of Maternal Vaccination and Its Effect on Vaccination Acceptance during Pregnancy

In England, influenza and pertussis vaccinations are recommended for pregnant women. However, in some areas, vaccination coverage rates are low. The aim of this study was to gain an in-depth understanding of how structural and sociopolitical factors influence attitudes toward and access to maternal vaccination. Participants were recruited in Hackney (a London borough), which has one of the lowest maternal vaccination coverage rates in the UK. In-depth interviews with pregnant/recently pregnant women explored experiences of care during pregnancy and their views on maternal vaccination. Interviews with healthcare professionals explored their views on maternal vaccination, and how they discussed it with patients. Data were analyzed thematically. Findings suggest that the currently popular individualist approach to healthcare places burdens on pregnant women that can negatively influence their vaccination decisions. Other structural factors, such as the state of the healthcare system, also influence women’s attitudes toward and access to vaccination, especially if they have certain intersecting identities.
