Evaluation of the factors that may contribute to the responsiveness of anti-epileptic treatments in pediatric patients, a study from Jordan

BACKGROUND Epilepsy is a common and debilitating neurological disorder. Disease prognosis depends on the quality of patient life with regard to its cognitive, behavioral, and social aspects and is not restricted to the frequency or severity of epileptic seizures. AIMS To recognize the seizure types in a Jordanian Arab population with pediatric epilepsy and to explore the clinical and medication-related factors that may affect the responsiveness and satisfaction with the treatments. METHODS A total of 300 pediatric patients (age range: 1-month to 14-years) with epilepsy were recruited. The patients were categorized into two groups, focal onset and generalized onset epilepsy. RESULTS The majority of patients (195) suffered from idiopathic epilepsy, and the majority were using only one medication, with valproic acid being the most frequently used. There was significant association between patient gender and the incidence of psychotic thoughts (P-value = 0.019), and between seizure type and the history of febrile seizures (P-value = 0.02). However, the numbers of psychotic patients (n = 13) and patients with history of febrile seizures (n = 25) were relatively small. Moreover, the number of medications used to control the seizures was significantly associated with the age of onset, type of epilepsy syndrome, and patients' response to the first trialed medication, with P-values of 0.006, 0.001, and 0.0001, respectively. CONCLUSIONS This is the first study comprehensively analyzed the clinical and demographic characteristics that affect therapeutic response of Jordanian-Arab epileptic and pediatric patients. Our results contribute to the understanding and improvement of current diagnostic and therapeutic procedures in epilepsy.
