The physical activity sector within the treatment of mental illness: A scoping review of the perceptions of healthcare professionals

Objective

The physical activity (PA) sector has an important role to play within the treatment of mental illness (MI). This review aims to summarize the literature examining healthcare professionals’ (HCPs) perceptions related to the PA sector to: 1) develop an understanding of the potential factors influencing collaboration with the PA sector in the promotion of PA for MI; and 2) identify gaps and areas for further research.

Methods

A structured search of PsycInfo, PubMed, CINAHL, and SPORTDiscus using terms related to key concepts such as ‘mental illness’, ‘health professional’, and ‘physical activity’ was performed up until February 24, 2020. Studies were included if they contained empirical findings describing HCPs' perceptions related to the PA sector within the context of PA promotion for MI. Documents were coded by six reviewers for study characteristics, design, professional, mental health condition, and findings.

Results

Fifty-one documents were included. Attitudes and beliefs were mainly positive with respect to PA sector involvement in the treatment of MI. Other HCPs have expressed an interest in collaboration with PA professionals and existing collaborative efforts have generally been viewed as benefitting other HCPs and individuals with MI; however, limitations in the findings preclude definitive claims. Some doubts about the value of collaborative efforts and tensions in role delineation were noted. Some HCPs were unclear about the role and scope of practice of PA professionals, lacked knowledge about existing referral options, expressed confusion about referral criteria and processes, and described uncertainty or limitations regarding PA professionals' training and experience with MI. Findings were limited and equivocal on the point of PA professionals' competencies to promote PA for MI and sparse research has considered views regarding PA professionals’ curriculum. Collaborative practices were uncommon, and few studies examined correlates or determinants of collaboration.

Conclusion

The literature examining health professionals' views of collaboration with the PA sector in mental health contexts is limited. While this review identified several issues consistent with the broader literature examining collaborative practice, there remains a need for focused inquiry into barriers, correlates, and determinants using established behavioural theories and models of collaboration. The development of future curriculum targeting PA professionals’ mental health competencies could benefit from a multidisciplinary perspective.
