Abstract Violence, aggression, and agitation are widely reported in both psychiatric and general hospital settings and are challenging clinical scenarios. A stepwise approach beginning with verbal de-escalation progressing to other measures such as medication, constant observation, and restraints is a standard approach with variable efficacy. Another approach to the management of agitation and aggression in the hospital setting is the inclusion of trained security dogs. Here we present a case in which inclusion of a trained dog assisted in crisis de-escalation. This case provides one example of the utility of such canine units. Future exploration including randomized control trials would help clarify the potential of canine units to enhance safety in acute psychiatric care and assess their cost-effectiveness.
