In this issue of the European Journal of Neurology (pages to be completed) , Rebordão et al reported 5 cases of acute ischemic stroke (AIS) presenting with chest or epigastric pain leading to the misdiagnosis of acute coronary syndrome (ACS) on initial examination. All the cases were extracted from the prospective and consecutive cohort of AIS, the Acute Stroke registry and Analysis of Lausanne (ASTRAL) over a 12‐year period, highlighting their very low frequency. As a result, there is a paucity of clinical and radiological data on this type of stroke chameleons in the literature.
