Commentary: Women With First-Hand Tobacco Smoke Exposure Have a Higher Likelihood of Having an Unruptured Intracranial Aneurysm Than Nonsmokers: A Nested Case-Control Study.

The authors1 present a nested case control study investigating the effects of smoking and hypertension on the prevalence of unruptured intracranial aneurysms (UIA) in female patients 30 to 60 yr of age. The patients had a brain magnetic resonance angiography (MRA) obtained between 2016 and 2018. In patients with a history of smoking, the prevalence of an UIA was 10-times higher (19% vs 1.9%) than for non-smokers. There was a significant association of smoking, hypertension, or a combination thereof with incidental UIAs. Smoking is often accompanied by hypertension, and both are known to be associated with UIA. While several previous studies have indicated a causal relationship between smoking or hypertension and intracranial aneurysm formation, an isolated analysis has rarely been performed. Therefore, the authors analyzed both their isolated and combined impact on the prevalence of UIAs and excluded patients with other known risk factors such as polycystic kidney disease or a family history of intracranial aneurysms.
