We read with interest the paper by Perosa et al. (2020) entitled ‘Hippocampal vascular reserve associated with cognitive performance and hippocampal volume’. The author reported that a mixed blood supply of the hippocampus by the posterior cerebral artery (PCA) and anterior choroidal artery (AChA) may provide vascular reserve and protect against cognitive impairment. The work is novel and critically important, given the relevance of hippocampal injury in Alzheimer’s dementia and other cognitive disorders. It is unclear, however, how the authors accounted for the heterogeneous configuration of the circle of Willis in their methods.
