How reasonable is it to uphold the diagnostic concept of schizoaffective disorder?

"A nosological nuisance, but a clinical reality", that is how psychiatrists Andreas Marneros and Hagop Akiskal once described schizoaffective disorder (SAD), aptly expressing a widely held view in the field and arguing for the necessity of the diagnosis. On the other hand, critics have emphasized weaknesses in the conception of schizoaffective disorder. Gin S. Malhi, to name only one of the most outspoken experts, called it "a total fabrication". The authors of DSM-5 and ICD-11, however, see a clinical need for they retained the diagnosis in their manuals, although in modified form, hoping to improve its supposedly low reliability.
