very high levels of a monoclonal ab that does not reflect the ab repertoire of patients with anti-NMDARe, and even less so, the abs of MPs. Second, there is no evidence that the abs of MPs are similar to those of patients with anti-NMDARe or have similar pathogenicity. One study assessing the effects of NMDAR-abs of asymptomatic persons, using high-resolution single particle imaging, showed that the abs were not pathogenic. Third, the scenario that best addresses the effects of placental transfer of NMDAR-abs from mother to fetus is provided by patients who develop anti-NMDARe during pregnancy or become pregnant soon after recovery. We reviewed our experience and that reported in the English literature (Table): (1) the neonatal mortality of children of mothers with anti-NMDARe during pregnancy was low; (2) if problems occurred, they were mainly due to obstetrical complications related to systemic symptoms of the mothers; and (3) during follow-up, most children appeared to be doing well. Thus, the findings suggest that children of mothers with anti-NMDARe do not have the high neonatal mortality or poor outcome suggested by Jurek’s model. On the other hand, the appropriate model to assess whether serum NMDAR-abs of asymptomatic mothers cause congenital neuropsychiatric disease in their children would be the use of IgG of these mothers instead of a monoclonal ab from a patient with anti-NMDARe.
