ABSTRACT We examined the utility of drugs versus urine to detect drugs of abuse by characterizing the number of hair and urine samples provided and the agreement between hair, urine, and self-reported drug use. Of 40 participants, 39 (97.5%) provided urine, whole only 31 (77.5%) agreed to provide hair (3 (7.5%) declined, 6 (15%) were not able). Hair detected more drug use than urine and self-report, and concordance between urine and self-report was 90% or greater. The advantage of hair to detect drug use, particularly cocaine, may warrant inclusion of hair samples to document cocaine or drug use during longer retrospective periods; if documenting recent use of drugs other than cocaine, self-report may suffice.
