In Advancing global health and strengthening the HIV response in the era of the Sustainable Development Goals: the International AIDS Society–Lancet Commission,1 it is emphasised that the risk for acquiring HIV, the occurrence of illness resulting from infection, and the risk of death are not solely determined by individual behaviours but are occasioned by the circumstances of people's lives—by social and structural determinants of health.2 What is counted as the social determinants of health in the global response to HIV has expanded in ways that were scarcely imaginable 20 years ago.
