For an outsider, the Lancet Psychiatry's Commission on the future of psychiatry1 allows a revealing insight into psychiatry's self-conceptualisation, the profession's ambitions, and its hopes. Globally, psychiatry seems to be experiencing a kind of identity crisis, which the Commission largely resolves to address through the vigorous scientific pursuit of the “clinical biomarkers [that] remain elusive”. In part, psychiatrists are constructed as brain doctors who are part of a helping profession, and with a desire not to be isolated from their physical-health colleagues.
