In the decade since Josep Dalmau described the first case series of anti-NMDA receptor encephalitis,1 the cellular mechanisms that give rise to autoantibody-associated encephalitides have been defined ever more accurately. By contrast, how immune responses disrupt brain systems that control emotional behaviour, memory, movement, and consciousness remains largely unknown. Substantial overlap between the symptoms of autoimmune encephalitis and psychiatric and neurological conditions continues to pose challenges for the rapid diagnosis and treatment of this potentially fatal disorder.
