William McDaniel and colleagues raise an important point concerning the uncertainty about what dose of ketamine should be combined with electroconvulsive therapy (ECT) when studying its potential benefit. In our study,1 we used a subanaesthetic dose (0·05mg/kg) that showed no benefit for either efficacy or protection of cognitive function, but we acknowledged that our data could not exclude beneficial effects from other ketamine doses or protocols.
