COPD exacerbator phenotype: time for reassessment?

A major goal in the management of chronic obstructive pulmonary disease (COPD) is to reduce exacerbations. Exacerbations are associated with increased mortality,1,2 worsened quality of life,3 and accelerated decline in lung function;4 making efforts to prevent these effects a worthy treatment goal. The landmark ECLIPSE study5 identified a group of patients with at least two exacerbations per year, which were classified as the frequent exacerbator phenotype. Since then, there has been a concerted effort to aggressively treat these patients in an effort to reduce exacerbations and improve outcomes.
