Multiple sclerosis tremor: are technical advances enough?

Tremor is the most common disorder in patients with multiple sclerosis and greatly contributes to the disability of these patients. This tremor rarely presents with the high-frequency action component seen in primary tremor disorders; rather, it is the combination of several features, particularly cerebellar ones. Therefore, drugs are rarely effective, whereas surgery can provide variable results depending on the severity of the underlying ataxia. In The Lancet Neurology, Seth Oliveria and colleagues1 report the findings from their single centre, single-blind, prospective, randomised pilot trial of dual-lead thalamic deep brain stimulation (DBS) in 12 patients with treatment-refractory tremor caused by multiple sclerosis.
