    Monkeypox virus (MPXV) is a large, double-stranded DNA virus belonging to the Orthopox genus in the family Poxviridae. First identified in 1958, MPXV has caused sporadic human outbreaks in central and west Africa, with a mortality rate between 1% and 10%.1 Viral genomes from west Africa and the Congo Basin separate into two clades, the latter being more virulent.2 Recently, MPXV outbreaks have occurred in Sudan (2005), the Republic of the Congo and Democratic Republic of the Congo (2009), and the Central African Republic (2016).

    In The Lancet Infectious Diseases, Timothy Walker and colleagues1 report on a cluster of 29 multidrug-resistant tuberculosis cases among asylum seekers arriving in Europe from the Horn of Africa. They use whole-genome sequencing to identify the transmission links and describe the cross-border collaboration and control measures. Their study is of interest as it sheds light on the importance of early tuberculosis detection and highlights the potential of wide application of molecular epidemiology techniques.

    Since April, 2016, Dengvaxia—a dengue vaccine produced by Sanofi Pasteur (Lyon, France)—has been licensed for use in 19 countries. Dengvaxia was recommended by the WHO Strategic Advisory Group of Experts (SAGE) on immunisation to be used in regions with high endemicity, as defined by a prevalence of dengue antibodies of more than 50% in the targeted age group of people aged 9–45 years.1 We have previously discussed the risks behind this vaccine recommendation,2,3 and by analysing an age-structured model4 using the available vaccine trial data1 predicted a significant reduction in dengue virus infection-related hospital admissions if Dengvaxia is given only to individuals who are seropositive for dengue antibodies.

  • Urgent challenges in implementing live attenuated influenza vaccine

    Conflicting reports have emerged about the effectiveness of the live attenuated influenza vaccine. The live attenuated influenza vaccine appears to protect particularly poorly against currently circulating H1N1 viruses that are derived from the 2009 pandemic H1N1 viruses. During the 2015–16 influenza season, when pandemic H1N1 was the predominant virus, studies from the USA reported a complete lack of effectiveness of the live vaccine in children. This finding led to a crucial decision in the USA to recommend that the live vaccine not be used in 2016–17 and to switch to the inactivated influenza vaccine. Other countries, including the UK, Canada, and Finland, however, have continued to recommend the use of the live vaccine. This policy divergence and uncertainty has far reaching implications for the entire global community, given the importance of the production capabilities of the live attenuated influenza vaccine for pandemic preparedness. In this Personal View, we discuss possible explanations for the observed reduced effectiveness of the live attenuated influenza vaccine and highlight the underpinning scientific questions. Further research to understand the reasons for these observations is essential to enable informed public health policy and commercial decisions about vaccine production and development in coming years.

    The 2002–03 outbreak of severe acute respiratory syndrome (SARS) is thought to have originated in horseshoe bats. However, because no strain in bats has been found that matches the one that infected people, uncertainty has existed about the exact origin of the outbreak. Now, genetic analysis of 11 newly discovered strains of SARS-related virus from bats in a cave in China shows all the essential building blocks needed to build the human SARS coronavirus, suggesting that recombination between precursor strains could have given rise to the 2002–03 strain of human SARS.

    2348 cases of plague (including confirmed, suspected, and probable cases) were reported by the Ministry of Health of Madagascar between Aug 1, and Nov 22, 2017, with 202 deaths. 1791 patients had pneumonic plague, with 22% confirmed, 34% probable, and 44% suspected. All contacts identified during the outbreak so far (7289) have completed a course of antibiotics; 11 developed symptoms compatible with plague and were classified as suspected cases. Control measures are to remain in place until the plague season finishes in April, 2018.

    The 2014–16 Ebola virus disease outbreak in west Africa was an unprecedented crisis that exposed weaknesses in health systems, outbreak response, and research. Poor preparedness caused delays in initiating research and publishing results. It is important to examine what happened and to learn lessons to improve in the future. Amanda Rojek and colleagues1 examined clinical research done during the epidemic and found 11 treatment trials registered with ClinicalTrials.gov or the ISRCTN registry, of which six are now published (one soon after the Review by Rojek and colleagues was published).

We acknowledge the authors' effort to update the current situation on this important issue. However, we cannot agree that most of the French Guianan population need to be vaccinated against this disease (ie, 200 000 people of a total population of 262 527, according to official estimates).3

    We read Delan Devakumar and colleagues' Review1 of infectious causes of microcephaly with interest. This Review describes the major congenital infections that cause microcephaly and substantial neurological impairment worldwide, focusing on pathogenesis, diagnosis, and management. As the Review discusses congenital infection (with reference to pitfalls in diagnosis), it is also worth noting the interferonopathies, also called Aicardi-Goutières syndrome. The interferonopathies are a spectrum of disorders that should be considered in infants with suspected congenital infection and negative results in laboratory tests, including for cytomegalovirus, rubella, herpes simplex virus, toxoplasmosis, and Zika virus.

  • The imperative of vaccination put into practice

    In an Editorial,1 The Lancet Infectious Diseases importantly addressed the topic of mandatory vaccination. Previously, Tony Kirby2 and Jonathan Kennedy and colleagues3 reported and commented on policy approaches adopted by Australia, Germany, and France in response to growing hesitancy around vaccination. Italy is among the several countries that have opted for mandatory vaccination. Italy's law 119/2017 makes ten vaccines mandatory for infants. These compulsory vaccinations are those against diphtheria, tetanus, poliomyelitis, hepatitis B virus (all of which were already compulsory) plus pertussis, Haemophilus influenzae type b, measles, mumps, rubella, and varicella.

Lancet Infect Dis 2017; 17: 1026–27—Incorrect terminology was used to refer to falsified medicines; this has been corrected throughout. The 2017 paper in Malaria Journal dealt with non-quality assured drugs, not inactive or adulterated drugs. Non-quality-assured medicines constitute 5% of the public sector in Kenya, not the private sector. Several of the quotes were taken out of context; they have been amended and a reference added. These changes have been made as of Nov 22, 2017.

    Each day of 2016, ten Chinese university students were infected with HIV—eight of whom were men who have sex with men (MSM).1 Unfortunately, in China, many MSM attending university first learn of HIV when they receive their HIV-positive test result. China has nearly 3000 universities and 40 million students. Of the male university students in China, 8·5% are MSM;2 thus, approximately 1·7 million university students are MSM aged 18 to 22 years.3 In this population, an increasing number of new HIV cases are reported.

    Congenital and perinatal neuropathology falls within two main phenotypes: primary microcephaly, a primarily grey matter disease observable at birth that is caused by defects in cell proliferation, axon guidance, or increased apoptosis; and secondary microcephaly, a grey matter or white matter disease of postnatal onset, caused by either a reduction in dendrites and synaptic connections, or defects in myelination, or even both mechanisms acting in concert (appendix).1,2 In this issue of The Lancet Infectious Diseases, Delan Devakumar and colleagues3 have seized on the opportunity of advances in Zika virus-related primary microcephaly to review the major infectious causes of microcephaly.
