When I came across the abstracts of the articles on “Hard Flaccid Syndrome—HFS” [1, 2] my first impression was that the articles were reporting a penile erectile deformation that I had described and named “Non-erecting Erection” more than 10 years ago in the 11th chapter of my book Text Atlas of Penile Surgery [3]. I have to admit that, later, after reading the entire articles I saw that I was wrong and understood that HFS is an erectile problem that I was not aware of and different than the pathologies I described. In their articles Gül et al.'s described an acquired erectile problem following vigorous masturbation or a kind of a penile trauma occurring during vigorous intercourse. Now I know that the erectile anomalies I described in my book chapter were totally different from HFS mainly because they were not acquired. Despite these differences, I thought that it will be appropriate to discuss both these rare penile anomalies for allowing proper diagnosis of each of them when other colleagues will see them in their practices.

My personal opinion is that there are several erectile problems that because of their rarity they do not attract the attention of the physicians, be a GP, a sexologist, or an urologist. In addition, they are either not described clearly or also they lack proper nomenclature to describe them creating a confusion.
