Surgical therapy as constant future gold standard for penile disorders

The field of urology has experienced dramatic changes in the last years, with respect to basic knowledge and therapeutic approaches. Novel medical options like chemotherapy and immunotherapy to treat various urological malignancies have been introduced and altered therapeutic regimens. Consequently, surgery in the uro-oncological field tends to decrease, and might be replaced, in part, by medical therapy in the future. In contrast, such advances in the sexual medicine and andrological field have been less evolving. This can be explained in part by the fact that there is not much support from industry or scientific and research organizations to push and drive basic and clinical research. Thus, surgical therapy will continue to be the gold standard to treat many diseases and conditions in this field.

Surgical therapy of various penile disorders has also changed significantly within the last 20 years. By means of increased anatomical knowledge, penile pathophysiology, as well as by the use of specialized and standardized surgical techniques we can reduce risks and complications to a minimum, and at the same time greatly improve surgical outcomes and patient satisfaction. Thus, the intention of this special issue of the International Journal of Impotence Research is to provide current, state-of-the art information on surgical therapy and techniques of five major penile disorders: erectile dysfunction, Peyronie’s disease, congenital penile curvature, penile fracture, and priapism. I thank all the authors that have contributed to this prominent work. All of them are world-wide recognized and renowned urologists, surgeons and experts in the field of penile reconstructive surgery.
