A 26-year study of restraint fatalities among children and adolescents in the United States: A failure of organizational structures and processes. Child & Youth Care Forum. https://doi.org/10.1007/s10566-021-09646-w that was published online on August 21, 2021. In that version, Table 2 Restraint fatalities by State and Time period lists New York State twice. The corrected Table 2 provided below has New York State listed once with the combined total of 5 fatalities with 2 fatalities between 1993 and 2005 and 3 fatalities between 2006 and 2018.

Under section Findings—Fatalities by State and Agency Type, the first sentence of the second paragraph now reads “The fatalities reported here occurred within 29 states.” This corrects New York’s duplicate count in the total states in the August 21, 2021 online version of the article.
