RE: neurogenic bladder is an independent risk factor for complications associated with inflatable penile prosthesis implantation

In recent years, we have witnessed a plethora of outcome studies evaluating the perioperative morbidities and complication rates in men undergoing penile prothesis implantation. The ultimate focus of these studies has been to better characterize clinical predictors, identify risk factors, and provide more effective patient counseling, subsequently improving surgical outcomes and satisfaction rates in men undergoing penile prosthesis implantation [1]. However, these studies have been hampered by the homogeneity or lack of outcome data specific to men at the highest risk, namely neurogenic bladder (NGB) individuals.

The harbinger of this recent interest has been the seminal work by Wilson et al., who demonstrated an increased risk of penile prosthesis infection in those individuals with spinal cord injury, compared to healthy individuals (9% vs 1%) [2]. Prior studies have determined the multifactorial etiology of increased infection complications in the NGB individuals include spinal cord injury, and noted a plausible linkage between preoperative urine cultures status and perioperative bladder drainage management strategies, such as clean intermittent self-catheterization vs indwelling Foley catheters vs suprapubic catheters.
