A 66-year-old man with a history of ischaemic cardiomyopathy and heart failure was referred to our institute for worsening shortness of breath (New York Heart Association III) and orthopnoea. Transoesophageal echocardiography (TOE) showed severe functional mitral regurgitation (MR; 3D EROA 80 mm2) with a centromedial origin of the regurgitant jet, a cleft posterior leaflet between scallop P2–P3 and a spotty fibrocalcific lesion on the distal body of the anterior leaflet (scallop A2) (Panels A–D, Supplementary data online, Video S1).
