Background: Emerging evidence suggests that the prosthetic design of implant-supported crowns, namely restorative angles may have an impact on peri-implant marginal bone remodeling, but studies supporting this hypothesis are lacking. Whether this hypothesis applies to Ti Bases remains unknown.

Aim/Hypothesis: To assess whether or not the restorative angle of implant-supported crowns on Ti Bases influences peri-implant marginal bone remodeling within 5 years of loading.

Material and Methods: Twenty-four patients with a single missing tooth in the aesthetic zone were recruited. All patients received a two-piece implant with a screw-retained veneered zirconia restoration extraorally cemented on a Ti Base abutment. Marginal bone levels (MBL) and clinical parameters including probing depth (PD), bleeding on probing (BOP) and plaque index (PI) were assessed at crown delivery (baseline), at one year (FU-1) and at five years (FU-5) of follow-up. To investigate the relationship between restorative angle and MBL, correlation tests and linear regression models were carried out. To assess the changes of MBL, PD, BOP and PI over time, the non-parametric tests Brunner-Langer and Wilcoxon signed-rank were performed.

Results: A total of 22 patients were available for re-examination at 5 years. The mean MBL amounted to 0.54 ± 0.39 mm at baseline and 0.24 ± 0.35 at FU-5 indicating a significant bone gain over time (p < 0.001). At FU-1, a moderate positive correlation (r = 0.5) between the mesial restorative angle and mesial MBL was found (p=0.012). Similarly, mesial MBL changes between baseline and FU-1 were also positively correlated (r = 0.4) with the mesial restorative angle (p=0.037). These positive correlations disappeared at FU-5 (p>0.05). Linear regression revealed that MBL (p=0.007) and MBL changes (p=0.03) were significantly associated with the mesial restorative angle at FU-1, but not at FU-5 (p>0.05). Logistic regression revealed that restorative angles >40° were less likely (OR: 0.1) to gain bone at FU-1. Conversely, no significant correlations between the distal restorative angle and distal MBL were found (p>0.05). At FU-5, PD increased significantly (p=0.04), whereas BOP and PI remained unchanged (p>0.05)

Conclusion and Clinical implications: The restorative angle of implant-supported crowns on Ti Bases influences the initial marginal bone remodeling at early time points (up to 1 year), but not thereafter. Considering an increasing use of Ti Bases, these findings provide guidelines for a correct prosthetic design for implant-supported crowns with Ti Bases.
