Background: Peri-implant keratinized mucosa (KM) is of great significance in the long term maintenance of restorations. Free gingival grafts (FGG) are considered as gold standard but have the limitations of poor color match and increased morbidity. A combination of FGG strip with xenogenic collagen matrix (XCM) was recently introduced showing favorable results in augmentation of KM with the advantages of reduced morbidity and better color match.

Aim/Hypothesis: To evaluate and compare the gain in peri-implant keratinized mucosa using a combination of FGG strip with XCM to that of FGG alone.

Material and Methods: In this randomized controlled clinical study, a total of 30 patients were included having minimal amount of peri-implant KM (≤1 mm). The study population was divided into 2 groups by randomization protocol; Group A (n = 15) received a combination of FGG strip and XCM and Group B (n = 15) received FGG only to augment KM. All the surgical procedures were carried out by a single operator by using a combination of FGG strip +XCM or FGG alone. Clinical parameters include width of keratinized mucosa (WKM) measured at baseline, 1 month and 6 months, pink esthetic score (PES) measured at baseline and 6 months and patient morbidity using visual analog scale (VAS) at day 1, day 7 and day 14.

Results: All the data was tabulated and analyzed using various parametric and non -parametric tests. . At 6 months both groups showed statistically significant gain in WKM when compared from baseline (p < 0.05). However this gain was found to be statistically non-significant when compared between the two groups at all time intervals, that is, at baseline, 1 month and 6 months (p > 0.05). On the other hand statistically significant difference was found between the two groups when the data of PES and VAS was compared (p < 0.05).

Conclusion and Clinical implications: The preliminary data obtained from this study indicates that the combination of FGG strip and XCM is a viable and predictable alternative in augmenting peri-implant keratinized mucosa with minimal morbidity and better esthetic outcomes when compared to the FGG which is considered as gold standard. However, long term studies with larger sample size are needed to confirm the results obtained in the present study to further validate this surgical procedure.
