Intramyocardial dissection of the left ventricle (LV) is extremely rare complication of myocardial infarction, trauma, aortic aneurysm, or may be iatrogenic. The diagnosis is difficult as it mimic left ventricular intraventricular thrombus. However, the distinction between these two complications is very important due to the differences in management and prognosis. The multimodality approach with computed tomography or magnetic resonance is difficult due to the usually associated haemodynamic instability. For this reason, echocardiography is the only exam to make a diagnosis.
