The authors1 discuss the phenomenon of heterotopic ossification (HO) following cervical total disc replacement (cTDR). They present a large, single-institution cohort of cTDR patients and conclude that the presence of preoperative ossification predisposes to postoperative HO. Furthermore, range of motion was significantly decreased, and adjacent segment disease was significantly increased in cTDR patients with high-grade HO. Although, at first blush, these findings seem intuitive, they provide the evidence basis for several of the central tenets of spinal arthroplasty, namely, the importance of patient selection in regard to successful cervical arthroplasty and that maintenance of motion plays a significant role in minimizing adjacent segment disease.
