Clinicians generally accept, almost without question, that patients undergoing elective laparoscopic cholecystectomy for symptomatic cholelithiasis will experience improvement in their symptoms after the operation. We believe this with such confidence that it remains one of the so-called safe places in general surgery, away from controversy, drama, or questionable expectations. In this issue of JAMA Surgery, Latenstein and colleagues1 have nicely shown that—of course—it gets complicated.
