Measuring the toll of Acute-Illness Derived Frailty (AIDF) in decompensated cirrhosis

Frailty is a biologic state of decreased physiologic reserve and increased vulnerability to stress, intrinsically linked to every aspect of life with decompensated cirrhosis. The catabolic nature of cirrhosis leads to insidious extra-hepatic effects such as muscle wasting, malnutrition, and functional impairments that compromise the health and wellbeing of each person affected. In the ambulatory setting, the chronic biologic state of decreased physiologic reserve and increased vulnerability to stressors has been operationalized using the Liver Frailty Index (LFI) and is now a well-accepted objective construct in hepatology and transplant care. The American Society of Transplantation (AST) endorsed the use of standardized frailty metrics including LFI for ambulatory liver transplant evaluation in 2019.
