A 27‐year‐old woman presented at a university hospital for right buttock pain (NRS:3) that has progressively increased in severity over 6 months. Patrick and sacral thrust tests elicited pain at the right sacroiliac joint (SIJ). The pelvis AP radiograph was unremarkable and computed tomography (CT) showed subtle focal erosion on the iliac side of the right SIJ (Figure 1). Magnetic resonance imaging (MRI) revealed bone marrow edema around the right SIJ. Mild periarticular edema was noted anterior to the joint.
