Dissociative Language Representation in a Patient with Schizencephaly

We present a 63-year-old, left-handed, healthy man who was identified as having schizencephaly [1] in the left hemisphere (Fig. 1), without any history of seizure or developmental problems. Comprehensive speech/language studies revealed mild dysarthria with limited lateral tongue movement and competent language ability except mild difficulty in repeating sentences. The data of fMRI showed the preponderant involvement of the right frontal in sentence completion and bilateral temporal areas in verb generation task (Fig. 2), indicating the dissociative and reversed intrahemispheric activation of language [2]. The developmental anomaly may have affected this peculiar language representation in the brain of this patient.
