ABSTRACT The author is to be congratulated on attempting to institute a discussion of an important subject. Unfortunately his paper does not do justice to the literature since it is severely incomplete. In this discussion first I will set out points where I agree with him, second those where I disagree and through that thirdly indicate some of the missing topics and researchers.
