Are we measuring what we ought to measure? A review of tools assessing patient perception of the healthcare built environment and their suitability for oncology spaces

Abstract Healthcare built environments are increasingly considered an important contributor to patient satisfaction and wellbeing. Nevertheless, few studies have developed rigorous survey tools to capture patients' experience of such settings, and hence facilitate investigating the nexus between the built environment and health-related outcomes, in a reliable manner. Even fewer consider ambulatory oncology settings, where patients usually undergo lengthy treatment sessions. Through systematic review of electronic databases including Ovid MEDLINE, CINAHL, PubMed and Embase, this article reviews eight tools developed to measure patient perception of healthcare built environments, with particular consideration of their capacity to assess the extent to which ambulatory cancer facilities possess the qualities that contribute to the creation of a supportive environment. Consideration of the efforts made to establish valid and reliable properties of such tools is also highlighted. The tools' reported psychometric properties are considered in the light of the recommended standards of the COSMIN checklist. Most of the detected tools exhibited shortcomings relevant to their construct, scope and theoretical foundation. The consequences of applying scientifically-rigorous testing methods on the effectiveness of these tools to comprehensively cover the architectural scope of healthcare settings is discussed. In sum, the review points to the need to develop tools with a stronger conceptual basis, to facilitate attaining rigorous properties of the tools’ constructs as well as ensure reliable description of patient experiences.
