1 BACKGROUND

Incompatible ABO blood transfusion may cause a severe intravascular hemolytic reaction that can be life threatening. In the case of transfusion of incompatible blood and the presence of a double population of red blood cells, establishing blood type is challenging. In addition to trauma cases and conflict regions, polytransfused patients may need to have their true phenotype resolved as part of the evaluation of alloimmunization.

During the past few years several trauma cases were admitted to our medical center from the Syrian civil war. Some of these cases may have been transfused with incompatible blood in Syria prior to their transfer to Israel (table describing cases available upon request). The double population of red blood cells observed in their blood samples upon arrival to our blood bank caused difficulties in determining accurate blood type of the patients by serology.

One of these cases was described recently when blood type was determined mistakenly by genotyping following an incompatible blood transfusion, resulting in further incompatible blood product transfusion (Akria et al., 2017). Thus, our laboratory sought a solution of blood typing using flow cytometry to determine the true recipient blood type rapidly and simply.
