Abstract Neurological side effects of tacrolimus have been well documented in the literature, while psychiatric symptomatology, especially mania, have rarely been reported. We report the case of a 64 year old man with no psychiatric history who developed mania with supratherapeutic serum concentrations of tacrolimus. The patient was treated with a mood stabilizer and an atypical antipsychotic while tacrolimus was discontinued and his other immunosuppressants were continued.
