Multiple medical or neurological conditions may mimic or contribute to a patient’s neuropsychiatric symptoms. In a retrospective chart review of 252 patients with a neurodegenerative disease who attended a specialty clinic, it was found that 28.2% of them received a prior psychiatric diagnosis (1). We herein present a clinical vignette involving an elderly Chinese lady with a historical diagnosis of bipolar disorder Type 1, admitted to us to our tertiary setting for a supposed relapse episode.
