We read with great interest this retrospective cohort study, which constitutes the largest series to date for unilateral moyamoya disease. In their article,1 the investigators evaluated natural history in 217 unilateral moyamoya disease patients while focusing on predictors of both contralateral radiographic progression and the need for contralateral revascularization over a long follow-up period of around 6 yr. This multiracial cohort of patients in North America included both idiopathic moyamoya disease and moyamoya syndrome patients. Only patients who already underwent bypass surgery on the affected side were included; consequently, the lack of conservatively managed patients precludes a complete description of natural history.
