We read with great interest this retrospective cohort study, which constitutes the largest series to date for unilateral moyamoya disease. In their article,¹ the investigators evaluated natural history in 217 unilateral moyamoya disease patients while focusing on predictors of both contralateral radiographic progression and the need for contralateral revascularization over a long follow-up period of around 6 yr. This multiracial cohort of patients in North America included both idiopathic moyamoya disease and moyamoya syndrome patients. Only patients who already underwent bypass surgery on the affected side were included; consequently, the lack of conservatively managed patients precludes a complete description of natural history.
