Six possible root causes were examined. Three were deemed unlikely, and three were either probable or likely root causes. In the end, electrostatic discharge appears to be the most likely root cause. The Case Study serves to remind scientists about the importance of keeping a clutter-free work area and about hazards associated with common reagents.
