Predicting microvascular invasion in HCC with ctDNA: What are the pitfalls?

We read with interest the paper “Preoperative evaluation of microvascular invasion with circulating tumor DNA in operable hepatocellular carcinoma” published in an upcoming issue of Liver International.1 It is an elegant study that looked at the association between microvascular invasion (MVI) and circular tumor DNA (ctDNA) for patients with surgical hepatocellular carcinoma (HCC) and their outcome prediction. Although the findings highlight the role of ctDNA in predicting the existence of MVI in HCC, there are some methodological flaws which may deserve the authors' attention.
